Wall Street closed for second day

NEW YORK (Reuters) - The stock market was closed for a second straight day on Tuesday as cash equity trading was canceled in the wake of Hurricane Sandy.

Tuesday's shutdown was the first time weather had resulted in a two-day market shutdown since the Great Blizzard of 1888. Exchanges expect to reopen on Wednesday.

NYSE Euronext said the New York Stock Exchange would open, although it will switch to fully electronic trading if necessary. Nasdaq OMX's Nasdaq Stock Market will also be operating on Wednesday, a source familiar with the matter said.

Index futures stopped trading electronically at 9:15 a.m. on Tuesday (1315 GMT) and ended the session largely flat. S&P index futures ended 0.2 percent higher.

Futures will reopen at 7 p.m. ET (2300 GMT) for the overnight session during European and Asian hours, closing again at 9:15 a.m. Wednesday morning.

Sandy, a mammoth storm, slammed into a large swathe of the densely populated U.S. eastern seaboard on Monday, forcing hundreds of thousands to seek higher ground, halting public transport and closing schools, businesses, financial markets and government departments. At least 18 people died and more than eight million homes and businesses were without power.

Investors expect heightened volatility when markets do reopen as the two-day closure creates pent-up demand.

Certain sectors are seen as especially tied to the fallout from the storm, which caused major flooding from storm surge during high tides, along with extensive damage from high winds and lashing rain. Disaster-modeling company Eqecat estimates Sandy caused between $10 billion and $20 billion in total economic damages, with $5 billion to $10 billion in insured losses.

Construction sectors as well as retailers such as Home Depot may see a boost from the eventual rebuilding effort, though airlines, which were forced to cancel thousands of flights, could see sharp falls. Insurance companies will also be in focus.

"With winds 75 miles per hour along with flooding in the most populated area of the country, you could have an unparalleled loss picture for insurance companies," said Joe Heider, a Cleveland based wealth manager at Rehmann.

Compounding the issue, Sandy arrived in the middle of the corporate earnings season. While some companies, including Pfizer Inc , delayed releasing their results until the storm passed, others released theirs on schedule, including Ford Motor and TD Ameritrade .

With their results out but investors unable to trade on them, those stocks may see particular interest on Wednesday.

He served as vice president and area general manager for The Ritz-Carlton Hotel Company in Dearborn, Mich., and as vice president and area general manager of The Ritz-Carlton hotels in Cleveland, St. Louis and Philadelphia before joining Henry Ford in 2006.

Here, Mr. van Grinsven explains how wellness services enable the hospital to care for the whole patient.

Question: Henry Ford West Bloomfield Hospital has several luxury services and amenities. What is your overall goal for these services?

Gerard van Grinsven: The overall goal for our diverse amount of wellness services is to answer the unexpressed wishes and needs of our customers ? who are our patients, their families, our employees and the community. I would like to clarify that our services are designed to enhance the patient experience and aid in the healing process. Our offerings are not luxury services; they are wellness services. Whether it is health coaching or acupuncture through Vita, the wellness center, a visit by our patient concierge, a cooking class designed to educate patients on their conditions or a visit to our therapeutic organic greenhouse ? they all have one purpose: to deliver an experience that transforms lives and communities through health and wellness, one person at a time.

The facility was built with the intention of taking health and healing beyond the boundaries of imagination by combining the best clinical care with the most innovative, comprehensive wellness philosophy, for our patients, their families and the communities we serve.

Q: How has your experience working at The Ritz Carlton influenced your vision for Henry Ford West Bloomfield Hospital?

GvG: It has allowed me to fully develop my skills in listening to the voice of the customer. Both industries are about developing relationships and creating memorable experiences for your customers. I also see many parallels between a hospital and a hotel. For instance, you dine at both places, stay at both places and sleep at both places. So why should the experience be all that much different? Our hospital is a destination for people that are ill or well. We want all our guests to feel like they want to come here, instead of feeling like they have to come here. To achieve this, we have offerings to enliven the senses and provide a healing environment, creating a community center for well-being.

The Ritz Carlton also is a talent-based organization. That is an element that I encouraged utilizing here, and it has been instrumental to our success. We use a scientifically based interviewing process that goes beyond skills and experience. It looks at natural talent. It looks past a resume, and instead searches for answers about an aptitude for compassionate care, ability to be an innovator and passion for thinking in different and unique ways. I believe building a talent-based staff is crucial to any organization, and the results show in our patient and staff engagement. As a two-time Baldrige winner with The Ritz Carlton Hotel Company, it only made sense to use the Baldrige criteria to help frame the leadership structures of our hospital, which are all centered around the customer.

But since I had never worked in healthcare, many people were interested in how an hotelier could make the transition. My first step was to surround myself with incredible clinical leaders. The Henry Ford Medical Group is one of the largest physician groups in the country. It is full of incredible talent and remarkable skills, which aided in my transition. In addition, we created relationships with outstanding private physicians in our community to complement the HFMG. Having the foundation of great clinical programs and services allowed me to utilize my talents in creating an environment of service excellence in surprising and delighting our customers. We created activities and programs to assist our communities in living healthier lives. We would redefine the delivery of healthcare by establishing a community center for well-being, bringing a new integration of clinical excellence, technology, patient safety and wellness programs to the Midwest and beyond.

This means we would need to build an incredible environment where healthy people want to be. Where they would want to take classes and participate in their health. We wanted an environment that would be a healing campus, surrounded by influences in nature. That included building the hospital with only private inpatient rooms. Studies have shown that not only does this help with the healing of the patient, but it reduces infection significantly. To add to this comfort, we eliminated overhead paging, and we do not wake our patients from 11 p.m. to 5 a.m. Examples like this, plus our outstanding clinical care, resulted in the customer being in the center of all that we do.

Q: How have the hospital's non-traditional services, such as yoga classes and fresh, to-order food, affected the hospital in terms of patient satisfaction, quality outcomes and finances?

GvG: To summarize, all of these innovative elements contribute to the overall patient experience and lead to market distinction and a loyal consumer base. But beyond that, it is the right thing to do. We are providing education for a healthier life. We are offering clinical excellence with holistic therapies to treat the mind, body and spirit. And our results show that these measures work.

Since opening, we have consistently been in the 99th percentile for Top Box (top score of 5) Likelihood to Recommend as compared to Southeast Michigan hospitals, [according to the] Press Ganey database. We believe these wellness services have helped us establish an environment where our patients truly feel we are caring for them holistically.

Our hospital became cash flow positive a year ahead of schedule. We have outperformed our net income budget every year since opening. We have increased volume every month since opening in March 2009, and increased market share each year in an economy that was in a downward spiral and in a market that is shrinking in population.

The decision to become a talent-based organization had a significant impact on reduced turnover, excellent employee engagement, reduction of harm, outstanding customer engagement scores and subsequently positive financial results.

Our organizational model has gone through extensive review in the last few years and continues to prove itself. First, in July 2010 Henry Ford West Bloomfield Hospital achieved ISO 9001:2008 certification. [The International Organization for Standardization] is an internationally recognized standards organization that establishes standards that ensure products/services are safe, reliable and of good quality. We continue to be surveyed for compliance with these standards annually, along with our [Det Norske Veritas] accreditation for hospitals. Secondly, we participated in the system-wide efforts and site visit that led to Henry Ford Health System winning the 2011 Malcolm Baldrige National Quality Award. To achieve this award, our leadership model, performance measures and clinical quality were scrutinized. In addition, they also reviewed and praised our efforts along the line of performance improvement and innovation.

Q: How does the hospital's approach to care ? including wellness-focused services and higher-end services ? align with healthcare reform goals of population health management and value-based care?

GvG: Many of the wellness-focused services are well aligned with the healthcare reform goals of population health management. Henry Ford West Bloomfield's approach to focusing on optimizing the health status of our patients, pre- and/or post-hospitalization, will help to reduce the overall cost of healthcare. For example, we have cooking classes specifically targeted to those individuals with diabetes, cancer and heart disease. We provide health coaching and exercise programs to enhance fitness and functional status. Essentially we inspire and assist patients in changing their lifestyles to improve the key risk factors which drive their chronic disease burden ? specifically healthy eating, regular exercise and focusing on being mindful of healthy lifestyle habits.

Furthermore, we are working on enhancements to care coordination and communication among the members of the care team. These enhancements include improvements in pre-hospitalization patient education, enhanced multidisciplinary rounds for inpatients, e-health visits and discharge phone calls ? all targeted at reducing readmission rates, which are directly tied to the value-based care initiatives. At Henry Ford West Bloomfield Hospital, the innovation starts with clinical care and continues into the realms of wellness and customer service.? ?

The patient experience at the West Bloomfield hospital is designed to meet the unexpressed wishes of patients and families during very stressful and dangerous health challenges that they face. While some would call our approach "luxurious," we don't see it that way. We believe that all patients and families deserve a compassionate and supportive care team that provides medical care in a healing environment. From a financial perspective, the environment we have created does not significantly impact the overall cost curve, because the amortized capital expense of a building is a very small component of the overall cost of care in an industry where labor expense makes up about 75 percent of overall cost.

Q: Can and should West Bloomfield Hospital's model of care be replicated in other hospitals and health systems, or does the model work only in certain communities and markets?

GvG: I encourage all healthcare institutions to embrace a patient-centered approach. We want to challenge the entire healthcare industry, in order to advance it through cutting-edge innovation. I do not believe this only works in certain markets or communities, the model can certainly be replicated, especially when you consider the services as wellness, not luxury, services. We have had numerous organizations come to benchmark us to understand what is at the core of both our clinical and wellness services. While every hospital and every market is different, the concept of focusing on our customers' overall health versus just their illness is universal. ?

At least 50 flooded houses destroyed by NYC fire

NEW YORK (AP) ? A huge fire destroyed at least 50 homes in a flooded neighborhood in the New York City borough of Queens.

More than 190 firefighters were trying to contain the blaze in the Breezy Point section and two people suffered minor injuries, a fire department spokesman said.

The fire was reported around 11 p.m. Monday in an area flooded by the superstorm that began sweeping through the city earlier, officials said.

Firefighters told WABC-TV that the water was chest high on the street, and they had to use a boat to make rescues. They said in one apartment home, about 25 people were trapped in an upstairs unit, and the two-story home next door was ablaze and setting fire to the apartment's roof. Firefighters climbed an awning to get to the trapped people and took them downstairs to a boat in the street.

Video footage of the scene shows a hellish swath of tightly packed homes fully engulfed in orange flames as firefighters hauled hoses while sloshing in ankle-high water. Many homes appeared completely flattened by the wind-whipped flames.

The neighborhood sits on the Rockaway peninsula jutting into the Atlantic Ocean.

In September, the beachfront neighborhood was struck by a tornado that hurled debris in the air, knocked out power and startled residents who once thought of twisters as a Midwestern phenomenon.

As much as Gmail shines when it's on the web, some of its most avid users stay in native apps for the multitasking; having to check a past message in the web client has usually meant putting the current draft on hold, or at least maintaining a near-photographic memory. Google wants to translate some of that desktop experience to the web through a new composition interface it's testing as of today. New messages start off in a shrinkable pop-up that lets us find old threads without having to put the new conversation on ice, even we're indecisive enough to leave multiple unfinished e-mail messages open. Other upgrades lurk in the background for the more focused among us, such as a pared-back composition interface, in-line photos and a reply box that dynamically adapts to the space it needs. Only those in the preview will see the Gmail update for now; Google is promising a wider launch in the months ahead that could save us all a few precious minutes each day.

How Hurricane Sandy became a snowstorm

Sandy just won't quit. The hurricane-turned-post-tropical-cyclone blew through the Caribbean last week, killing at least 69 people, most of them in Haiti and Cuba. Sandy then churned up the U.S. East Coast, making landfall Monday night in New Jersey and bringing massive floods to Atlantic City, Manhattan and other coastal areas.

And now Sandy is a snowstorm.

Thanks to a collision with cold air flowing from the Arctic, the post-tropical storm (so called because it has moved out of tropical latitudes), has so far dumped 26 inches (66 centimeters) of snow on Redhouse, Md., 24 inches (61 cm) in Alpine Lake, W.Va., and 18 inches (46 cm) in Newfound Gap, Tenn., according to The Weather Channel's Mike Bettes.

The National Weather Service (NWS) has issued blizzard warnings for high-elevation areas of the central Appalachians, and a swath of lower elevation areas from western Maryland southwest into eastern Tennessee and western North Carolina are under winter storm warnings and advisories. Sandy's remnants could drop up to 3 feet (0.9 meters) of snow in parts of West Virginia and up to 2 feet (0.6 m) in southwestern Virginia and eastern Kentucky before moving into Canada on Wednesday, according to the NWS. The transformation of Sandy from rainstorm to snowstorm is a consequence of a blast of Arctic air that has fed the storm even as it has moved out of tropical waters. Normally, a tropical storm like Sandy would weaken as it moves northward into cooler waters. But a dip in the polar jet stream has fueled the storm, prompting some to call it a "nor'eastercane," a combination of hurricane and nor'easter. Nor'easters are storms driven by frigid polar air. (The moniker "Frankenstorm" also stems from this weather mash-up.)

This polar air on Sandy's western side is turning rain to snow ? a situation that's not entirely unprecedented.

According to the Washington Post's Capital Weather Gang, a "snowicane" hit upstate New York and New England in 1804, and 1962's Hurricane Ginny dumped snow in Maine. In 2005, the Category 5 storm Wilma fed into a nor'easter that dropped up to 20 inches (50 cm) of snow in West Virginia and New England.

Monica?s Famous Baked Macaroni and Cheese

Serves:?6-9

Ingredients

* 1 box pasta of your choice that is tubular and has ridges (holds sauce better)

* 4 tablespoons butter

* 3 tablespoons flour

* 1 tablespoon ground mustard

* 1 teaspoon paprika

* 2 tablespoons minced onion

* 2 cups half and half

* 2 large eggs

* 8 ounces shredded sharp cheddar cheese (reserve 3 ounces for topping)

* 8 ounces shredded Italian blend cheese (reserve 3 ounces for topping)

* 6 ounces havarti cheese

* ?seasoning salt

* fresh cracked black pepper

Topping:

* 2 tablespoons butter

* 1 1/2 cups panko bread crumbs

Directions

Preheat oven to 350 degrees F.

In a large pot of boiling, seasoned water (season your water for pasta always with Extra Virgin Olive Oil, garlic powder, onion powder, and seasoning salt), cook the pasta til still slightly firm.

While the pasta is cooking, chop onion. In a separate pot, melt the butter. Whisk in the flour and seasonings over medium high heat and keep it moving for about five minutes, making sure it is free of lumps - this removes the raw flour flavor. Slowly stir in the half and half. Simmer for 5 minutes. Remove from heat.

Next, temper in the eggs. Stir in cheeses (reserve 2 ounces each of cheddar and Italian blend for topping). Season with salt and pepper to taste. Fold the pasta into the mix and pour into a 13x9x2 casserole dish. Top with remaining cheese.

Panko Topping:

Melt the butter and stir in the panko bread crumbs to coat. Sprinkle the macaroni with the bread crumbs. Bake for 15-20 minutes - just until the top is golden brown and the cheeses have melted -- overbaking may cause eggy texture. Remove from oven and rest for five minutes before serving.

NOTE: I usually double this recipe and put it in a lasagna dish (deep dish casserole) -- it is 9x13x3 and holds it fine. This feeds a crowd or you can make 2 13x9x2 dishes and give one away! When giving away, I leave the final bake for the recipient.

Greece considered more risky to invest than Syria

LONDON (AP) ? The world's markets may believe that the worst of the financial crisis in Europe is over after three turbulent years, but those people who control the purse strings of the world's businesses are not breathing any easier.

An annual survey of finance directors from global business consultancy BDO finds that the crisis over too much government debt in Europe remains one of their key concerns ? so much so that Greece is considered a riskier place to invest and set up business in than war-torn Syria.

Only Iran and Iraq are considered more risky than Greece, which also struggles to convince its international creditors that it deserves bailout loans to avoid bankruptcy and a possible euro exit.

"CFOs are becoming increasingly wary of Southern Europe, parts of which they now see as risky as the politically unstable countries of the Middle East," said BDO chief executive Martin Van Roekel.

Greece isn't the only country in the 17-country group that uses the euro in the survey's top 10 riskiest countries to invest in. Spain, which even as the eurozone's No. 4 economy with a long-standing relationship with Latin America, stands at No. 7.

This reluctance by finance directors, particularly from fast-growing economies such as Brazil and China, to invest in Europe's indebted countries goes to the heart of the financial crisis. A major part of these countries' recovery is dependent on the private sector stepping in to fill the investment gap left by cuts in government spending.

While countries like Greece and Spain are struggling to convince international business that they are good places to invest, others are prospering. Despite recent signs of slowing down, China is considered the most attractive country for expansion, closely followed by the U.S. Others such as Brazil, India, Germany and the U.K. also feature in the top 10 of countries ripe for expansion.

Overall, the survey from BDO found that CFOs around the world are finding it more difficult to conduct business abroad. As well as an uncertain global economic situation, they cite increased regulation and greater competition.

Van Roekel also said he is "surprised" that more finance directors haven't voiced concerns about the heavy debts of countries outside of Europe, notably Japan and the U.S.

Though Japan's debt is worth around double the size of its economy, the country has managed to avoid stoking too many investor concerns because most of its self-financed by its own pension funds.

The U.S., which has the advantage of having the dollar, the world's reserve currency, has problems of its own and the winner of the presidential election, whoever it is, will soon have to grapple with the "fiscal cliff" ? a package of huge tax hikes and spending cuts that will automatically be introduced if the different arms of government don't come to a budget agreement.

Last week, Cathy Derus, the Mint.com Home Economics Sweepstakes winner, shared her before and after pictures of her TV room makeover. Today, Cathy is back with her favorite tips and tricks to making your DIY home makeover a success.

Back in July, MintLife hosted its Home Economics sweepstakes. As the lucky winner, I received a $400 gift card to West Elm and 1-hour phone consultation with designer Tracy Hutson. I was beyond excited to win and couldn?t wait to start decorating!

Tracy had some great design advice for us and we are so happy with how our TV room turned out. In addition to Tracy?s decorating tips, we learned a few other things in the process:

Be patient

You might think a $400 gift card and professional help would guarantee immediate design success, but decorating takes time! It takes time to pull together inspiration and then even more time to turn your ideas into a reality.

If you?re looking for something on Craigslist, be prepared to wait even longer. I love West Elm, but I didn?t want our room to look like a carbon copy page from their catalog ? I wanted to put my own touches on the design.

Plus, I didn?t want to settle on something that was just ?good enough.? We?ve learned to be patient and wait until we find the right item for our space.

Live with your room

My husband and I are ?have to see it? people. Yes, looking at magazines or Pinterest helps, but actually seeing an item in person and in our space is much more helpful.

For example, while shopping at West Elm, I thought their bone inlay Parsons mirror would be perfect for the bathroom adjoining the back room. When I brought the mirror home, it looked good in the back room, but seeing the mirror in the space made me realize it was destined for a completely different room.

Also, when we first talked with Tracy, we thought we would rather have side tables over a coffee table. But as we lived with our room, we realized we needed a place to put a drink and kick up our feet. So, a coffee table or ottoman went back on the list.

Get creative

A great way to (sometimes) save money and decorate with a custom piece is to create it yourself. After looking at several stores and websites, we decided to build our own upholstered coffee table. That?s an adventure in and of itself, but when it?s complete, I?ll be happy knowing it?s one of a kind.

DIY takes time

Our back room is actually home to several DIY projects: the TV unit and shelves, entry table, art, and coffee table. But unlike Extreme Makeover: Home Edition, our home makeover is being completed by an army of just two weekend warriors.

There were times when Mother Nature put a damper on our plans to spray paint, or we had to endure downtime while waiting for something to dry. So unless you?re willing to put in the extra effort, sometimes it?s worth it to outsource.

(AP) ? Republican presidential candidate Mitt Romney and President Barack Obama frantically sought to close the deal with voters with precious few days left in an incredibly close race as this year's October surprise ? an unprecedented storm menacing the East Coast ? wreaked havoc on their best-laid plans.

Ever mindful of his narrow path to the requisite 270 electoral votes, Romney looked to expand his map, weighing an intensified effort in traditionally left-leaning Minnesota. Obama sought to defend historically Democratic turf as the race tightened heading into the final week.

Wary of being seen as putting their political pursuits ahead of public safety, the two White House hopefuls reshuffled their campaign plans as the storm approached. Both candidates were loath to forfeit face time with voters in battleground states like Virginia that are likely to be afflicted when Hurricane Sandy, a winter storm and a cold front collide to form a freak hybrid storm.

"The storm will throw havoc into the race," said Sen. Mark Warner, D-Va.

Before leaving Washington for Florida Sunday, a day early to beat the storm, Obama got an update from disaster relief officials before speaking by phone to affected governors and mayors.

"Anything they need, we will be there," Obama said. "And we are going to cut through red tape. We are not going to get bogged down with a lot of rules. We want to make sure that we are anticipating and leaning forward."

An opportunity for Obama to demonstrate steady leadership in the face of crisis was offset by the risk that the federal government, as in past emergencies, could be faulted for an ineffective response, with the president left to take the fall.

"My first priority has to be making sure that everything is in place" to help those affected by the storm, Obama told campaign workers Sunday in Orlando.

He told the volunteers they would have to "carry the ball" while he was off the campaign trail.

"I hate to put the burden of the entire world on you, but basically it's all up to you," he joked.

Obama will hold a rally in Orlando on Monday with former President Bill Clinton, but he canceled campaign stops in Virginia and Ohio on Monday and in Colorado on Tuesday. He planned to return to Ohio on Wednesday with stops in Cincinnati and Akron, followed by a Thursday swing through Springfield, Ohio, Boulder, Colo., and Las Vegas.

Romney nixed three stops in up-for-grabs Virginia on Sunday, opting instead to campaign with running mate Paul Ryan in Ohio before heading Monday to Wisconsin, where Romney has chipped away at Obama's lead.

"I know that right now some people in the country are a little nervous about a storm about to hit the coast, and our thoughts and prayers are with people who will find themselves in harm's way," Romney told several hundred supporters crowded into a field house at the University of Findlay, the second of three Sunday rallies.

Romney's campaign confirmed Sunday that he would not travel to New Hampshire on Tuesday as planned. The campaign already canceled a Monday event in New Hampshire featuring Romney's wife, Ann. Advisers say further travel changes are likely as they monitor the storm's progress.

Vice President Joe Biden canceled a Monday event in New Hampshire. "The last thing the president and I want to do is get in the way of anything. The most important thing is health and safety," Biden said.

Ryan planned to leave Ohio at midday for three stops in Florida. His Tuesday schedule, however, shifted him to stops in Colorado instead of Virginia.

The prospect that bad weather could hinder early voting and get-out-the-vote efforts is vexing to both Obama and Romney.

"Obviously, we want unfettered access to the polls, because we think the more people that come out, the better we're going to do," said David Axelrod, a top adviser to Obama's campaign. "To the extent that it makes it harder, that's a source of concern."

In Virginia, one of the most competitive states in the race, election officials eased absentee voting requirements for those affected by the storm.

"The state board of elections is already planning for extended hours in advance for absentee voting, and it's now a priority, moved up to the same level as hospitals and police stations to have power restored," said Gov. Bob McDonnell, a top Romney ally.

Bringing up a safety concern, Virginia Senate candidate Tim Kaine's campaign urged supporters to remove their political yard signs. "Due to the potential for strong winds in this storm, the last thing we want is for yard signs to become projectiles," said campaign manager Mike Henry.

Getting voters to the polls ? whether early or on Election Day ? is one of the few elements of the race still in the candidates' control. Although Romney and Obama are in a close contest for the popular vote, the president continues to have the upper hand in the most contested states.

Reince Priebus, the GOP chairman, pointed to recent gains for Romney that have lifted him to a virtual tie in most national polls. "The challenger always wins in a tie race," he said.

With time running out, both campaigns appeared to be fine-tuning their map of the states where they're campaigning the hardest.

A senior Republican official, speaking on the condition of anonymity to disclose private deliberations, said Sunday that the Romney team was seriously discussing sending Romney, Ryan or both to Minnesota during the final week. The state hasn't gone Republican in the presidential race since 1972, but recent polling shows a tighter race there than most anticipated.

In a flashback to the 2008 race, Obama's campaign announced that Biden will campaign Thursday in Pennsylvania, reprising a visit to his hometown of Scranton that he made during the final week four years ago. Pennsylvania, too, has been Democratic territory in recent years, but Romney has continued to contest the state with an advertising assist from the Republican Party.

Axelrod, Priebus and McDonnell spoke on CNN's "State of the Union." Warner spoke on "Fox News Sunday."
